I understand my Champion's physical fitness will be put to the test during their Test-Drive, and in order for them to participate in track and field offered by Blue Lightning Track Club, it is required that I provide a current sports physical examination approving them to participate by a qualified healthcare provider. This provider can be a doctor, nurse practitioner, or physician assistant.
